By signing up for a squash membership with the club, I understand I am also applying to become a member of Squash New Zealand Poipātū Aotearoa and the District Association which the club is affiliated to.
Where I am applying for membership as a parent or guardian of a minor, the application is on behalf of the minor and does not entitle me to membership in my personal capacity.
I agree that my personal information (and, where applicable, the personal information of the minor for whom I am the parent or guardian) will be shared with Squash New Zealand Poipātū Aotearoa through the creation of a personalised MySquash account for the purposes of,
- Providing me (and, where applicable, the minor for whom I am the parent or guardian) with access to the features associated with the relevant MySquash subscription which forms part of the benefits of club membership
- Maintaining the national squash player database
- Statistical and administrative reporting
